1. Designated Laser Safety Officer (LSO)

ANSI Z136.3 — the standard governing healthcare laser safety — requires every facility operating a Class IV laser to designate a Laser Safety Officer. The LSO is accountable for establishing and enforcing the facility's laser safety program. In a small med spa, this is often the owner or lead practitioner. 2. Wavelength-Matched Optical Protection

Every person in the treatment room — operator, client, and any observer — requires protective eyewear matched to the specific wavelength(s) of the laser in use. A diode hair removal laser operating at 808 nm requires goggles rated for 808 nm. A Q-switched Nd:YAG operating at both 1064 nm and 532 nm requires protection across both wavelengths. Proper laser safety eyewear is one of the lowest-cost line items in a safety build-out — and one of the most commonly skipped.

3. Controlled Access and Warning Signage

The treatment room must be designated a Nominal Hazard Zone (NHZ) with controlled access during operation. This means:

  • Laser-specific warning signage on every door that may provide access to the treatment room
  • A lockout or controlled-entry protocol during active treatments
  • Window covers or beam stops to prevent specular reflections from exiting the room

4. Written Laser Safety Program

A written Laser Safety Program (sometimes called a Laser Control Program) documents how your facility manages laser hazards. At minimum, it should include:

  • Device inventory with make, model, wavelength, and class
  • Written standard operating procedures (SOPs) for each device
  • Incident and near-miss reporting procedure
  • Operator training records
  • PPE inventory and inspection log

This document becomes important if your facility is ever inspected by a state agency or medical board — and it's the first thing your malpractice insurer will ask for in the event of a laser-related adverse event.

5. Operator Training and Certification Records

Equipment ownership does not confer operator competency. In Arizona, Texas, Nevada, and most Southwest states, operating an aesthetic laser without appropriate training and credentials exposes your business to regulatory action and civil liability. Keep signed training records for every operator on staff, including the date of training, the device covered, and the credentialing body.
